Payment Options - Convenient ways to pay your plan

Paying plan installment dues

Plan installments may be paid at any of EPI branches, accredited banks, collection partners and licensed sales associates.

Current billing statement or installment due notice is required in paying at accredited banks and collection partners. For planholders paying outside the country, they may use RCBC Telemoney facility. Paying at these facilities will require that the plan is on active status. The deposit slip or the machine validated installment due notice serves as official receipt of payments made.

Licensed sales associates are also authorized to accept installment payments from their planholders. A provisional receipt shall be given and the Official Receipt is issued after remittance to any of the branches. (If no Official Receipt is received within 30 days, please confirm your payment with our Customer Service Unit).

Planholders may inquire on the status of their plan and/or on payments made with the Customer Service Unit.

Plan Amendments

Updating of information on issued plan contracts

Changes in the plan data (example: change of scholar or beneficiary) may be made at any time before the maturity date or availment of the plan. A new policy contract, if necessary, shall be issued to reflect the requested changes.

Requirements:

  1. The plan (life, pension or education) has to be on active (not lapsed) status.
  2. Certificate of Full Payment, if fully paid
  3. Duly accomplished Request for Special Transaction Form (available at the branches)
  4. Original Policy Contract
  5. Valid ID of the representative and Notarized Special Power of Attorney, signed by the planholder, indicating the name of the representative and specific authority granted.(for transactions through a Representative)
  6. Payment of Processing Fee

Note: Change of billing address may be requested by accomplishing the form included in the billing statement without need of the above-cited documents.

Transacting on various concerns through a representative

Planholders may transact with Eternal Plans on various concerns through a representative. However, the planholder himself/herself has to sign the Request for Special Transaction Form.

Requirements:

  1. Duly accomplished Request for Special Transaction Form signed by the planholder.
  2. Notarized Special Power of Attorney, signed by the planholder, indicating the name of the representative and specific authority granted.
  3. Photocopy of any Valid ID of Planholder and representative
  4. All other required documents and/or payments pertinent to the requested transaction.

Reinstatement of Lapsed Plans

Reinstating a lapsed plan

A plan is considered lapsed if the installment due is not paid within 60 days after due date. It can be reinstated at any time within two (2) years.

Two ways to reinstate a lapsed plan:

  1. Reinstatement by Updating:
    All past due installments plus penalty interest have to be paid. The maturity date as stated in the original contract remains the same.
  2. Reinstatement by Re-dating:
    This requires payment of only one (1) installment without penalty interest, but the maturity date will be moved to a later date and a new contract will be issued.


Requirements:

  1. Duly accomplished Request For Special Transaction Form
  2. Accomplished Insurance Health Declaration Form (2 copies)
  3. Applicable installment amount plus interest, if any
  4. Original policy contract for Reinstatement by Re-dating
  5. Photocopy of any valid ID
  6. Valid ID of the representative and Notarized Special Power of Attorney, signed by the planholder, indicating the name of the representative and specific authority granted (for transactions through a Representative)
  7. Reinstatement Fee


Considerations in Reinstating Your Plan:

  1. The payment and required documents have to be submitted at any of the branches or authorized representative.
  2. Submission of required documents and payment do not immediately reinstate the plan. Reinstatement of a lapsed plan requires the approval of the Eternal Plans, Inc.
  3. For insurance purposes, additional documents may be required.
  4. Planholders of reinstated plans will have to undergo one (1) year contestability period.


Planholders may contact EPI’s Conservation Department for queries regarding the status of their application for reinstatement.

Transfer of Ownership

Transferring ownership of a pre-need plan

Subject to approval of Eternal Plans, Inc., a pre-need plan (life, pension and education plan) may be transferred (sold or donated) to any eligible individual at anytime before the maturity date of the plan.

Requirements:

  1. The plan (life, pension or education) has to be on active (not lapsed) status.
  2. Duly accomplished Request for Special Transaction Form (available at the branches)
  3. Duly accomplished Pre-need Application Form of transferee.
  4. Original Policy Contract
  5. Certificate of Full Payment for fully paid plans
  6. Photocopy of birth certificate of new scholar (for education plan)
  7. Photocopy of any Valid ID of planholder and transferee.
  8. Valid ID of the representative and Notarized Special Power of Attorney, signed by the planholder, indicating the name of the representative and specific authority granted.(for transactions through a Representative)
  9. Payment of Processing Fee


Planholders may contact Conservation Department for assistance in transferring a pre-need plan to another eligible person.

Termination

Termination of plan and availment of Guaranteed Cash Value

Planholders may terminate their plan and avail themselves of the Guaranteed Cash Value at any time during the validity of the plan, subject to pre-determined values at the time the plan is terminated.

Requirements:

  1. Original Policy Contract
  2. Updated plan installment payments
  3. Certificate of Full Payment for a fully paid plan
  4. Request letter for Termination (to avail of the guaranteed cash value)
  5. Current Community Tax Certificate and two (2) valid IDs


Note: The date of surrender pertains to the date the request is submitted to Eternal Plans, Inc. Upon receipt of the Guaranteed Cash Value, the contract is deemed terminated.

For assistance in terminating a plan, planholders are advised to contact any of EPI’s branches of Claims and Availment Department.

Pension Benefit Availment - Claiming plan maturity benefits and requirements

Availment of Pension plan Benefits.

The maturity benefits of a pension plan are available on the date specified in the policy contract.

Pension Plan Requirements:

  1. Original Policy Contract
  2. Certificate of Full Payment
  3. Letter of Intent
  4. Current Community Tax Certificate and Two (2) valid IDs of planholder /claimant
  5. Affidavit of Guardianship (for minor beneficiary/ies)


Additional Requirements for Insurance Claim:

  1. Notarized claimant’s statement
  2. Notarized attending physician statement
  3. Certified True Copy of Medical Records
  4. Certified True Copy of Death Certificate
  5. Birth or Baptismal Certificate of the Deceased Planholder
  6. Three (3) Legal Documents bearing the deceased’s signature
  7. Birth or Baptismal Certificate/s of designated beneficiary/ies
  8. Marriage Contract of female married beneficiary/ies
  9. Police Report and other pertinent documents, if death is due to accident or violent death


All claims for insurance benefits are subject to the approval of Eternal Plans’ insurance carrier, Fortune Life. The pertinent documents have to be submitted and depending on the evaluation of the insurance company, additional forms may be requested.

Life Availment - Claiming of life plan benefits and requirements

Arranging a memorial service (inclusive of assignment cases):

Our service is just a phone call away. And one phone call does it all.

Things to do when the need arises:

  1. Call Eternal Plans immediately.
    • You may call our memorial service hot line number (02) 706-3442, the branch or authorized representative. You may also call the following Memorial Service Administrators: Ronald Centeno – (0917) 526 7345 Anthony Joven – (0917) 893 0633
    • Don’t contact or sign any document with any funeral home.
    • Eternal Plans will make the arrangement with a funeral home of choice.
  2. Relay the following information:
    • Name of caller, contact number and relationship to the deceased
    • Policy Contract Number
    • Name of deceased planholder or assignee
    • Exact address or location of the deceased
  3. Wait for the Memorial Service Administrator of Eternal Plans to handle the case.
  4. Eternal Plans, Inc. will contact the authorized representative of the bereaved to make the arrangement for the memorial service.
  5. An accredited funeral home will pick-up the body and render the memorial services.
    Requirements:
    • Full payment of unpaid installments, if any
    • Original Policy Contract
    • Certificate of Full Payment, for fully paid plan
    • Death Certificate
    • Assignment Form (for cases of assignment)

Education Benefit Availment - Availing education plan benefits and requirements

Availment of Education plan Benefits.

  3. Registration Form/Assessment/Curriculum
  4. 2 pcs. 2 x 2 colored picture of scholar
  5. Current Community Tax Certificate and Two (2) valid IDs of planholder/claimant
  6. Affidavit of Guardianship (for minor beneficiary/ies)
